Pain and Sensory Management

Medications that numb or reduce pain in the oral cavity.

Benzocaine

Benzocaine is a widely used local anesthetic, serving as the active ingredient in several over-the-counter pain relief products such as:

  • Anbesol

  • Kank+a

  • Orabase B

  • Orajel

These products are effective for topical pain relief, making them popular for treating oral discomfort. Benzocaine, when combined with antipyrine, forms A/B Otic Drops (sold as Auralgan), which are used for:

  • Relieving earaches

  • Softening and removing earwax buildup

How Benzocaine Works Benzocaine creates a chemical barrier by preventing sodium from entering free nerve endings. This blocks nerve depolarization and the resulting action potential, which means the brain cannot interpret sensations like pain in the affected area.

Potential Side Effects

  • Mild Reactions: Allergic responses like redness or irritation.

  • Serious Complications: In rare cases, benzocaine can cause methemoglobinemia, a condition where oxidized hemoglobin reduces oxygen levels in the blood, leading to tissue hypoxia.

Fluoride

Fluoride is a vital mineral that strengthens teeth and prevents tooth decay. It occurs naturally in water and many foods and is crucial for maintaining oral health by:

  • Reversing early decay

  • Making teeth resistant to acid damage caused by plaque buildup

Fluoride for Children Children under six may benefit from fluoride, which integrates into the development of permanent teeth, creating a stronger foundation against decay.

Sources of Fluoride

  • Many toothpastes and mouthwashes contain fluoride.

  • Tap water in many major cities is fluoridated.

  • Prescription fluoride: Dentists and doctors can provide stronger fluoride treatments in the form of:

    • Tablets

    • Oral rinses

    • Gels, foams, or varnishes applied directly to teeth

Who May Benefit Most from Fluoride

  • Patients with xerostomia (dry mouth) or gum disease

  • Those with a history of frequent cavities

  • Individuals with crowns, bridges, or braces
